About Our Team
On December 4, 2019, Robin Martin, Parliamentary Assistant to the Minister of Health, on behalf of Christine Elliott, Deputy Premier and Minister of Health announced the North Western Toronto Ontario Health Team as one of the first OHTs selected to implement a new model of care that is more connected and accessible.
Our Partnerships
The partnership currently has 14 signatory partners across the health sectors.
The sectors include primary care, secondary care, rehabilitation and complex care, home and community support services, mental health and addictions, and residential care.
Our Population
Our OHT serves over 414,000 people within our attributed population and community through our partner organizations. We are responsible for people receiving care in the North and West regions of Toronto.
Taking into account the many different and diverse needs and communities we serve, our OHT has identified, prioritized, and embedded health equity initiatives across our many initiatives. We have a dedicated Health Equity Committee that is committed to ensuring we develop and offer equitable care across our partner organizations. This means that our NWT OHT community can expect to receive high quality care.
Our Priorities
The NWT OHT is concentrating its’ efforts on improving population health.
Specifically, we have begun initiatives to provide care to patients with chronic conditions that have a significant prevalence in the community including congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD) and mental health and addictions – schizophrenia, psychotic disorder, and substance use disorder.
In addition to this, our OHT continues to leverage many different digital platforms to better integrate the care we provide to our community. As our journey continues, we will champion initiatives that relate to mental health and addictions, health equity, chronic conditions, and COVID-19 recovery efforts.
