How does it work?

1. Tell us about your needs

Complete the online intake form to tell us about your health needs and the supports you may be looking for.

2. We find the right supports

A care navigator will review your information to help you find primary care and other helpful services in your community.

3. Get connected to care

When a primary care match is found, you will get a letter with information on how to book your first appointment. For other supports, someone may call you to help link you to the right services.

Open to anyone looking for a family doctor or nurse practitioner in North Western Toronto – with or without OHIP. North Western Toronto postal codes start with: M3J, M3K, M3L, M3M, M3N, M6A, M6B, M6L, M6M, M9L, M9M, M9N, M9P, or M9R.
