We’re excited to be your one-stop shop for community services right here in our neighborhood! At The HUB@2115, we understand that navigating your healthcare journey can be a bit overwhelming. That’s why we’re here to help simplify things for you.

The North Western Toronto Ontario Health Team has brought together health and social care partners all in one place. Our goal is to make your access to services easier, with better coordination, shorter wait times, and improved outcomes. Whether you need help coordinating your care or just some extra support, we’ve got you covered.

Accessing Services at the HUB

All of our services are free and you don’t need an OHIP card. We offer a mix of by-appointment, by-referral, and some walk-in options. Our services and hours can vary, so please refer to the list below for the most current information. Different services are offered during different days and hours.

Address

We’re conveniently located on the second floor of 2115 Finch Ave. West (Suite 204 – Building C). There’s pedestrian access, wheel trans access, and paid parking is available behind the building. We cover parking for clients so please ensure you bring your parking ticket to the front desk to be validated.

Medical Emergencies

Please remember, The HUB@2115 is not a walk-in medical clinic and does not offer emergency medical services. For emergencies, call 9-1-1 or head to the nearest emergency department.

No, unfortunately we do not offer primary care services at the HUB. If you are looking for a family doctor or nurse practitioner please use Health Care Connect.
