Health Care Connect is a publicly funded program in Ontario that helps residents without a family doctor or nurse practitioner find one who is accepting new patients.

Who can register?
You must be a resident of Ontario with a valid OHIP card.
How do I sign up?
Register online and complete a health questionnaire to help determine your health care needs or by dialing 811.
What happens next?
Once registered, you’re assigned a Care Connector, a nurse from Ontario Health atHome. They work with local family physicians, nurse practitioners, and primary care teams to match you with a provider.
Getting matched
Priority is given to individuals with greater health needs. Once a match is found, you’ll receive a letter with the provider’s contact information so you can book your first appointment.
