The HUB@2115 offers a number of programs and services right in your community. The HUB is open from Monday to Friday. Check out the services below and when they are available to clients.
Jane/Finch Centre – Service Navigation Program
The Service Navigation Program supports residents of Jane and Finch and of Northwest Toronto in accessing mental health services and social supports in their immediate communities. It is open to anyone ages 13+ who are experiencing or are at risk of negative mental health outcomes. Upon registration to the program, a dedicated Community Wellbeing Worker will engage individually with clients and their caregivers to co-create a service plan that holistically addresses clients’ mental health and wellbeing. Beginning with a warm handoff followed by regular follow-ups, the worker will coordinate with different service providers to ensure that clients successfully navigate and access needed supports.

Black Creek Community Health Centre – Healthcare Navigation Services
Clients receive assistance to access BCCHC’s allied healthcare services, referrals to community services, forms completion, appointment booking, and accessing government supports (ODSP, OW, ADP).

Across Boundaries – Intensive Case Management Services
Across Boundaries provides equitable, inclusive and holistic mental health and addiction services for racialized people across the Greater Toronto Area. Across Boundaries acknowledges the impact of racism and anti-Black-racism on mental health. We believe in empowering individuals to take control of their health in their healing journeys.

Black Creek Community Health Centre – Mental Health Counselling
Clients can access private and confidential counselling from registered social workers and therapists to address anxiety, depression, and other mental health concerns.

COSTI Employment Services – 1:1 Employment Services
Career Consultants work 1:1 with clients to explore their career options, develop employment goals, support with job searching, assistance with finding employment. Learn about suitability requirements for job coaching, Better Jobs Ontario Training, on-the-job training including apprenticeship, job placement support, and other specialized employment & community services. In addition, COSTI provides Job Search workshops focused on résumés & cover letters writing or practicing interview skills.

Toronto Employment Social Services (TESS) – Ontario Works Emergency Support Program
Ontario Works in an emergency support program that helps individuals dealing with challenges like poverty, unemployment, homelessness, addiction, literacy challenges, mental health concerns, and settlement needs. Ontario Works offers money to help individuals and eligible family members with living expenses, including food, and rent, health benefits, employment supports in partnership with employment Ontario. TESS provides support with submitting applications and facilitate warm transfers and/or referrals.

Working Women Community Centre – Settlement Services
Clients (immigrant and refugee women and their families) can receive settlement information and support in such areas as: temporary or permanent housing; child care services and public education for children; English language classes; employment services; government services (social insurance number, health card, etc.); legal services and legal aid; access to health services; training and adult education programs; and completion and assistance with immigration forms and processes.

Ebeano Newcomers Network (ENN) – Refugee Immigrant Community Services
Access free, confidential support including counseling, information and resources, referrals, and connections to settlement services, immigrant communities, and an immigration lawyer.

Black Creek Community Health Centre – Diabetes Care, Management & Education Services
Clients with pre-diabetes and Type 2 diabetes receive education from a registered diabetes nurse. A nutritionist provides healthy eating education to prevent the development and advancement of the disease. If necessary, a chiropodist can assess foot health for diabetes-related foot issues, as well as other foot conditions and injuries.

Black Creek Community Health Centre – Nutrition Services
Clients receive help with digestive disorders, high cholesterol, pregnancy nutrition, food allergies, infant, toddler and child nutrition, meal planning and weight management.

Reconnect Community Services – Health and Wellness Program
The Health and Wellness program promotes a healthy and active lifestyle for older adults and seniors aged 55+ by offering programs that facilitates appropriate exercises and meaningful social and recreational activities in a safe, supportive, inclusive and fun atmosphere.
