Pre-registration is required to access this service.
1. Self-referral: Call 416-249-8000 or 416-246-2388 to register.
2. Referral through Primary Care or a Nurse Practitioner: Healthcare professionals can send a referral to People Accessing Care Teams (PACT) via this referral form.
