Have you discussed making a plan for the person you support?

Advance care planning and goals of care discussions involves talking with the person you support about their healthcare wishes before they get very sick. This can include decisions on the kinds of medical treatments and/or future care settings they want.

For example, does the person you support wish to move to a long-term care or retirement home? Do they wish to remain at home for as long as it is safe to do so? By having these conversations, you will be able to plan and make decisions as their needs change, while ensuring their wishes are met.

It‘s never too early to discuss the person you support wishes for care. These conversations will be different based on whether the person you support is living with an illness or not.
