Interested in joining the NWT PCN?

This year, Ontario Health announced their support of collaboration and engagement with primary care in Ontario through the formation of Primary Care Networks (PCNs). PCNs are distinct structures comprised of Primary Care Physicians, Nurse Practitioners, Interprofessional Healthcare Clinicians and Patient Family Caregiver representatives.

The PCN will provide strategic input and information on the OHT’s overall approach to OHT service delivery and planning as it relates to primary care.
